To apply to The Registry at a reduced rate you can fill out your
application on-line. After you have submitted your application, send in your payment and all necessary documentation to the Registry
office. |

Copy of Official transcript(s) from institute(s) of higher education (if
applicable) q
Verification of
degree completion (if applicable.) q Registry verification labels Send only original labels. Copies will not be accepted (not all training is verified using labels). q Continuing Education Form If you are documenting training using a continuing education form, the form must be signed by director or presenter of training. Unsigned education training forms will not be accepted q Payment Check or money order made payable to The Registry. If you are a certified or family child care provider from Milwaukee County do not send any payment. q High
school graduation or GED date in the Personal Information section q Identification
Number Either your Social Security number or Registry Identification
number in the Personnel Information section q Start Date Write in date you began your current position on Position Information section q Signature
You must sign the back page of application (application will not be
