Certificate Reprint Request
If your certificate expires within 6 months, you will need to renew your certificate.
We are unable to issue reprints for certificates printed before October 1,2008.

Entry Level Postcard Reprint Request
If your course was completed before July 2000, contact the agency where you completed the course to request another copy. The Registry does not have Entry Level records prior to July 2000.
Do not use this form if your course was completed at a college/university or in high school. The Registry cannot create these documents.
